Patient Information


Office Hours: Our office and telephone lines are open from 8:00am to 5:00pm Monday, Tuesday, Thursday. We are open 8:00am to noon on Wednesday and 8:00am to 2:00pm on Friday. We are closed Saturday and Sunday.

Making an Appointment: When making an appointment please inform our staff about your specific need so that we can appropriate the right amount of time for your visit. We attempt to schedule appointments to address our patients’ medical needs. You should bring a picture identification and your insurance card with you to your first visit. Whenever your insurance changes, we should also be provided a copy of the new card.  Pregnant patient should notify us as soon as they are aware that they may lose their insurance, we may be able to help you through the transition.

Canceling an Appointment: If you are unable to come at your scheduled time, please give us at least 24 hours of notice. Missed appointments or appointments cancelled less than 24 hours of your scheduled time will be charged $35.00. In addition, if you are unavoidably late for your appointment, please inform us of the situation. If you arrive more than 30 minutes late, we may need to reschedule your appointment.

Fees and Payments: Our fees are based on the usual and customary professional fees for obstetrics and gynecology in the Huntsville, AL area. For services not covered by insurance, we accept cash, VISA and MasterCard, CARECREDIT. For other payment arrangements, please see our office billing staff prior to your appointment.

Confidentiality: Your medical information is strictly confidential. We will not release it to anyone without your written consent. A family member may, however, accompany you to your appointments if you wish.  We assume that any information released to whomever you bring into the room, has been consented to, by you. If you want a copy of your records or yourself or to be sent to another physician, we will require a written authorization from you.
